I follow this polemic within the confort of my European culture, where this concept of first Lady is quasi inexistent because the only thing that matter is the person who’s elected, not her/his relatives.

In France were I live, the first Lady could be a Dr of a Professor or whatever she wants, and her title would remains the same.

She doesn’t even have to marry or have children, in fact, the president doesn’t even have to have a spouse to be elected, because we couldnt care less (even if we still think it’s funny when our president pays a secret visit to his mistress riding a motorcycle).
